Sunrise Hospital Cardiovascular Nurse Navigator
Our Cardiovascular Nurse Navigator works closely with the physicians in our various Heart Programs here at Sunrise Hospital and Medical Center, coordinating with the physicians and patients for the best possible outcomes.
As an example, our Nurse Navigator will provide education to patients related to their illness and any planned procedures. The Nurse Navigator’s goal while working closely with the physicians is to provide individual care plans for the development and enhancement of the patient’s overall experience.
